Job Description: Provide evaluations for companionship, personal care and light housekeeping services to individual clients of Cornerstone Helping Hands in the customer’s home.
To develop a plan of care appropriate for the client by Cornerstone Helping Hands.
To communicate to your supervisor any changes or concerns regarding a client’s health and/or the appropriateness of services being delivered.
To use good judgment in decision-making and to always respect confidential matters relating to a client.
To positively represent Cornerstone Helping Hands through attitude, appearance, work manner, timeliness and relationship with each customer.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Arrive at each assignment on time and complete the tasks outlined in the plan of care developed for each client. Tasks assigned may include, but are not limited to, the following.
- Evaluation of client needs and in accordance with client preferences
- Evaluate home environment and client care needs to maintain client and MHH staff safety.
- Convey concise services available and document in service agreement to client.
- Develop written in-home service plan for each individual client.
- Communicate service needs to companion staff of Cornerstone Helping Hands.
- Make Supervisory visits as assigned.
- Provide support to Manager of Cornerstone Helping Hands as needed.
- Assigns duties as needed.
- Assists in training of companion staff as assigned.
- Works and communicates with MSSOC and OMH personnel as required.
- Other duties as assigned
SUPERVISORY RESPONSIBILITIES:
Supervisory visits of Cornerstone Helping Hands companions as needed and assigned.
CERTIFICATES, LICENSES, REGISTRATIONS: Must have a current LPN license from the Ohio Board of Nursing and a pharmacology card from either N.A.P.N.E.S. or O.O.P.N.E.S. or an O.B.N. card
Valid driver’s license
Acceptable motor vehicle driving history
